For PT, Occupational Health, Walk-In & DOT / FAA Clinics

The modern patient check-in system your clinic deserves.

Patients sign in on a tablet, your front desk hears a chime, the queue updates live, and every visit becomes an audit-ready record. Built for physical therapy, occupational health, walk-ins, and DOT / FMCSA collection sites — one platform, every clinic.

FAA & FMCSA-aware 49 CFR §40 aligned Audit-ready records No PHI stored
Who it's for

One check-in system. Every kind of clinic.

If patients walk through your door and you need to know who showed up, when, and why — CollectorBench fits.

Physical Therapy

Patients check in for their session on the lobby tablet. The PT sees the queue update, knows who's next, and every visit is logged for billing and progress notes.

Occupational Health

Pre-employment physicals, return-to-work clearances, and injury follow-ups — all triaged from one queue. Employer contact captured at check-in.

Walk-In & Urgent Care

No appointment? No problem. Patients enter their reason for visit, and your front desk sees them appear on screen with a soft chime — no clipboard required.

DOT / FMCSA Collection Sites

All six reasons under 49 CFR Part 40: pre-employment, random, post-accident, reasonable suspicion, return-to-duty, follow-up. Plus DOT physicals and FAA flows.

Everything you need

A complete check-in & compliance toolkit.

From the QR scan on the wall to the signed certificate in their inbox — CollectorBench handles every step your clinic runs today, just faster and without paper.

Branded QR check-in

Print a QR code, stick it on your front desk. Patients scan, fill out their info, and land in your queue — no app to download, no clipboard to chase.

Real-time queue

Live dashboard showing who's waiting, who's testing, who's done. Color-coded by test type — DOT, FAA, pre-employment, random, post-accident.

Auto-generated certificates

PDF certificates with QR verification, signature blocks, and FAA/FMCSA-ready fields. Generated the moment the test is marked complete and emailed automatically.

Role-based access

Front desk, collector, lab tech, DER, and owner — each role sees exactly what they need and nothing more. Multi-tenant isolation between locations and companies.

Audit trail, always-on

Every check-in, signature, and status change timestamped and signed. Export the full audit log as a PDF for your next DOT or FAA inspection — in one click.

SMS & email notifications

Auto-notify the patient when it's their turn, the employer when the test is complete, and the DER when results come back. No more "did you get my paperwork?" calls.

How it works

Three steps from paper chaos to paperless calm.

Set up your site

We onboard your clinic in under an hour — your logo, your collectors, your test types, your QR codes printed and ready for the front desk.

Patients self check-in

Patients scan the QR code or use the lobby tablet. Employers send their people a direct link. Everything flows into a single live queue.

Test, sign, certify

Your collector finishes the test, signs in-app, and the certificate is generated and emailed automatically — with audit-ready records preserved forever.

Built by the developer, used at a real clinic

I built it for the collection site I work at. Now it powers three production workflows there.

I’m a full-stack developer at a South Florida collection site that runs FAA and FMCSA testing every day. I designed and coded CollectorBench from scratch — the database, the worker, the dashboard, the print pipeline, all of it — so we could stop chasing paper and clipboards.

Three workflows at the clinic are already powered by CollectorBench: donor check-in, the training portal our DOT clients certify on, and the DER assistance flow. When you sign up, you get the same platform I use myself — hardened by real audits, real walk-ins, and real DOT paperwork.

Talk to us
Trusted by compliance teams

Built by someone who runs a collection site.

CollectorBench grew out of a real clinic in Florida that runs both physical therapy and DOT collections. Every feature exists because we needed it ourselves.

"
We went from 40 minutes per intake with paper forms to under 8 with the QR check-in. My collectors actually have time to do their job now.

Our last FMCSA audit took 6 weeks of paper-pulling. With CollectorBench, the auditor asked for our chain-of-custody log and I had a signed PDF in their inbox in 90 seconds.

The branded portal is what closed it for me. My patients see our logo, not some white-label generic. Looks like we built our own software.

Common questions

Everything else you're wondering.

Is CollectorBench HIPAA-compliant?
HIPAA doesn’t apply to CollectorBench because we never store protected health information. We collect only donor name, employer, reason for test, and signature — no Social Security numbers, dates of birth, medical results, or insurance data. PHI stays in your existing clinic systems (paper files or EMR), so you don’t need a Business Associate Agreement with us, and there’s nothing for HIPAA to regulate.
How long does setup take?
Free guided setup is included on every tier and takes up to 2 weeks — most clinics are fully live within a few business days. Starter plans are self-serve and you can run your first check-in within 15 minutes of signing up. Professional and Enterprise include a guided onboarding call, staff training, and help importing your existing donor and employer lists.
Does it only work for DOT and FAA, or can a regular clinic use it?
Both. CollectorBench was built around 49 CFR Part 40 (DOT/FMCSA) and 14 CFR Part 120 (FAA) workflows — all six test reasons included — but the same check-in and queue work for any clinic: physical therapy, occupational health, walk-in, urgent care. Pick the reasons that match your clinic at setup.
Can patients check in without an app?
Absolutely — that's the whole point. The QR code opens a mobile-optimized web form in their browser. No app downloads, no accounts, no friction. You can also place a tablet in the lobby for walk-ins.
What if I already use other software?
Professional and Enterprise plans include CSV import/export. Enterprise unlocks a REST API so you can connect to Quest, LabCorp, your EHR, your billing system, or whatever else you run.
What payment methods do you accept?
All major credit cards via Stripe. Enterprise customers can pay by ACH or invoice (Net 30).
